Obtaining Baseline Vital Signs (BERPS)

Acronym: B.E.R.P.S.
  • B
    Blood Pressure
    1. Apply the sphygmomanometer.
    2. Inflate the cuff.
    3. Auscultate.
    4. Note findings.
  • E
    Eyes
    Pupillary response:
    • Equal/unequal
    • Reactive (to light)
    • Note: in case of normal findings, verbalize "Pupils are equal and reactive."
  • R
    Respirations
    Count respirations for 30 seconds, then multiply by two.
    Note any issues with respirations.
    Note: a single respiration consists of chest rise and fall.
  • P
    Pulse
    Find the radial pulse and count the beats for 30 seconds, then multiply by two.
    Note any issues with the pulses.
  • S
    Skin
    Assess color, temperature and condition of the skin.
